• ON THE DAY OF...

             

             On the day of Tuesday, August 29, 2017. 

     

    • BRING THE MAZE DAY PACKETMake sure all documents (see list of REQUIRED FORMS below) are completely filled out and signed as applicable before arriving and standing in line. Parents/legal guardians (current grades 1-5 only) who submitted their forms received during Open House in May 16, you will not need to complete the forms again. For those that did not pick up their packets and all incoming TK, K, and 1st grade students, the packets will be sent to their residence a week prior to the scheduled MAZE Day, Tuesday, August 29, 2017. 
    • Classroom assignments will be released to Parent/Legal Guardian only. Bring one current government issued ID (i.e. Driver’s License, DMV ID, and Passport)
    • Only assigned Grade Levels and Alpha (if applicable) will be serviced during the scheduled time.
    • MAZE Day ends promptly at 2 PM. Late registrants will need to return tomorrow, 1st day of school, before 8:30 AM in Room 101. After 2 PM on Maze Day, the staff will return to their regular assignments in preparation for the 1st day of school.

             After visiting the designated area for classroom assignment distribution, parents/legal guardians are welcome to join the festivities at

             Gomes Hall/Multi-use room. There will be tables for:

     

    • Parents and Teachers Association (PTA) - Join Gomes PTA today! 
    • Bay Area Child Care (located at Gomes grounds) - Need childcare at a convenient location? Talk to us!

             Below is the list of forms required to be submitted on MAZE Day. Parents/legal guardians (2016-2017 grades 1-5 only) received the Registration Packet as early as May 16, Open House at Gomes.     Review "How to Complete the Forms" *before* completing each form to ensure you are filling it out correctly.

    • DISASTER RELEASE INFORMATION FORM (3-ply document, do *not* separate these forms after completing; Only original form sent to you by Gomes Office will be accepted.)

     

    • FUSD TECHNOLOGY FORM (1 white sheet, back-to-back; Can be printed using attachment below.)

     

    • OPTIONAL OPT-OUT FORM DIRECTORY INFORMATION (1 white sheet; Can be printed using attachment below.)
