    Q: When is Open Enrollment this year and how do I participate?

    A: Open Enrollment is September 1, 2018 through September 30, 2018 for dental and/or vision plans, with an effective date of October 1, 2018. Open Enrollment for PERS health plans is September 10, 2018 through October 5, 2018 with an effective date of January 1, 2019. During Open Enrollment, you may enroll in, change, or delete health/dental/vision plans or add eligible family members who are not currently enrolled. Similar to last year, this year all forms will be sent via email through Informed K12 (previously Chalk) with a link to the forms. The forms can be filled out, signed electronically, and submitted with a single click.

    Q: Do I have to use online forms; can I still send the paper application forms?

    A: Open Enrollment flyers will be sent to all active qualified employees in addition to the emailed Open Enrollment notification. We will still accept the paper forms, but encourage the use of the online application form. The online format will allow for more efficient processing of the many transactions we receive during open enrollment. The enrollee will also receive an email confirmation of the submission to use as a receipt. If there is missing or incorrect information on the submitted forms, they can quickly be emailed back to the enrollee, thus eliminating a trip to HR to fix the form/add missing signatures.

    Q: What about dependent verification documents; I am adding my spouse/child is there a way to upload the documents?

    A: The dependent verification documents (birth certificates, marriage certificates, etc.) can be uploaded through the online enrollment system. At the bottom of the page is an “Attachments” button. You may upload your previously scanned documents by using this button. Documents may also be submitted in person in HR, sent through inter-office mail, or faxed/emailed to HR Benefits staff. Copies are acceptable. It is your responsibility to ensure your dependent verifications have been received. There is a 30-day grace period for providing verification documents. Coverage is not guaranteed after the 30-day grace period expires.

    Q: Why is there a different open enrollment and effective date for health coverage as opposed to dental and vision coverage?

    A: The open enrollment period and effective date for health insurance is set by CalPERS; we are bound by contract with CalPERS to follow the dates they have determined. The dental and vision open enrollment periods are pre-determined by the respective plans, with an effective date of October 1st.

    Q: I do not want to make any changes to my current coverage. Do I have to do anything?

    A: Yes. Open the email from Informed K12. View your form. Select Step 1. Keep my current coverage. Submit. Easy!
