     Instructions for Obtaining Use of School Facilities

    1. Facilities Use Permit can be obtained at any school/Community Services Offices at 4210 Technology Drive, Room 230, Fremont, CA 94538 (510-668-1609) or download form.

    2. Complete APPLICANT’s portion of the Facilities Use Permit.
    3. Take facilities use permit and $15.00 application fee to School for completion of SCHOOL’s portion by the principal who will pre-approve the dates of use and custodial support needs.
    4. Obtain and submit with the Facilities Use Permit a Certificate of General Liability Insurance for $1,000,000 and naming Fremont Unified School District as additional insured.

    5. Contact City of Fremont if you have 300 or more participants for you may need a special event permit.

    6. Facilities Use Permit is then forwarded to Community Services Office for final approval or denial.  Insurance requirements will be checked, fees will be ascertained.  Completed Facilities Use Permit must be received by Community Services 15 working days prior to the event.  Copies will be distributed to all concerned.

    7. Approved Facilities Use Permit must be on hand during use for inspection purposes.

    8. To add additional hours, dates or facilities, a new Facilities Use Permit is required and application fee applied.  Any other change,Facilities Use Permit Change Form must be completed and submitted with $15 change fee five working days prior to the event. Verbal changes or changes made via email are not accepted.

    9. Applicants are billed base from the facilities use permit – room rent and custodial hours.

    10. Applicants will be billed for any damage or vandalism caused by their use of school facilities.

     


    SMOKING AND ALCOHOL ARE NOT PERMITTED AT ANY TIME ON ANY PROPERTY OWNED BY FREMONT UNIFIED SCHOOL DISTRICT


    ALL FACILITIES USE PERMITS MUST FIRST BE SIGNED FOR PRE-APPROVAL AT THE SCHOOL CONFIRMING OR DENYING DATES AND TIME OF USE.  Community Services cannot approve dates and times of use.

    Request for use of facilities by a non-school organization requires prior approval from Community Services.  Approval is not completed until a facilities use permit number has been assigned by Community Services.

    INSURANCE: A CERTIFICATE OF GENERAL LIABILITY INSURANCE MUST BE IN THE COMMUNITY SERVICES OFFICE BEFORE A FACILITIES USE PERMIT CAN BE FINALIZED. If insurance requirements are not met, the facilities use permit will be denied and the facility cannot be used.
