The entire packet, stapled and in order, should consist of:

  1. All students need:
    1. CCCapply.org confirmation page
Go to website, click apply, continue, new user, make sure you enroll for "Ohlone Community College" and when you get to the end there will be a confirmation code - print that page! You should receive an Ohlone Student ID via email within a few days. **
      Sample Confirmation Page:
      CCCApply
    2. Ohlone College Application [pdf]

For #12 Intended Academic Program, write "ND.K12"
For #18, if you are taking another Ohlone course (like a foreign language), you may go over 7 units and there's another form you need to obtain from Chris Williamson at Ohlone's Registrar
    3. Parent/Guardian Authorization Form for Minors (aka Health Waiver) [pdf]
  2. 9th Grade Students need:
    1. K-9th Grade Exceptional Student Form [pdf]
Page 2: requested class synonym number is provided by your teacher
  3. 10th-12th Grade Students need:
    1. 10th-12th Grade School Permission Form [pdf]

* According to K-9 Special Students Application Information [pdf], "K-12 students are exempt from paying the California Community College Enrollment Fee ($20 per unit), but are subject to a $16 Health Services Fee ($12 during Summer Term), a $5 Online Registration Access Fee, and a $5 ($2.50 Summer) optional Student Activities Fee, unless all classes are off-campus." This means you *may* receive a charge of $5 or $21, but this hasn't always been enforced in previous semesters, so you may not receive a charge at all. If this is a hardship, please contact me ASAP.

**Once you've obtained an Ohlone student ID via email, you may officially register for your Ohlone class via https://webadvisor.ohlone.edu
