Athletic Form Needed to Play at AHS

Athletes wishing to participate in any sport need to submit an athletic clearance form found below to the Athletic Director. For WINTER sports, this form must be returned on or by Monday November 7, 2011.

Print out two copies of this form, one for the athlete"s files, and one for the coach to keep at all times.

Student FORM

1. Fill out all information (All one Form)

a. Student Information

b. Sport trying out for (if multiple sport indicate all sports)

c. Parent /guardian Information

d. Parent /guardian Consent to play Must be signed.

e. Evidence of Physical Must have

f. Medical information Must have

g. Download and print AHS Athletic Department Clearance Form

2. Money

a. Checks made out to FUSD $50; AHS $30 (memo students name)

b. Cash (we prefer NO cash) Please use an envelope sealed with your name and sport on envelope

3. Turn all documents and pament to your COACH
