Current Student Reservation

Current Student Reservation 2012-2013

All current students, Kinder through Grade 5, will receive a blue reservation form in the mail. The forms will be mailed the week of February 11th, 2013. When you receive the form, fill it out and return it to the school office.

You must submit two proofs of residency. If you are residing with someone else and cannot provide the two proofs as outlined below (requires a Declaration of Residency), you must attach or bring in two pieces of current business mail addressed to you at your residence

Property tax payment receipts

Rental porperty contract, lease, or payment receipts

Utility service contract, statement, or payment receipts

Pay stubs

Voter-Registration

Correspondence from a government agency

Documents provided as proofs must be current within the past 60 days.
