Thanks for volunteering to drive on a field trip!!
PLEASE READ EACH STEP CAREFULLY. IF YOU CAN FOLLOW THESE DIRECTIONS THE FIRST TIME, IT WILL SAVE ME THE TIME OF RESENDING THE INFORMATION TO YOU FOR MULTIPLE FOLLOW-UPS. I APPRECIATE THAT.
 
To drive on a field trip I must have all your paperwork on file in the office at least a week before the field trip. It is a long process, so the sooner you get started the easier it is for me!
 
Step 1: Print the Declaration of Volunteer Driver form found at this link to the FUSD website.
 
Step 2: Fill in all the information. Be sure that your insurance meets the minimum coverage amounts.
Important: NO child may ride in the front seat of any car. When filling in the Passenger Capacity on this form, be sure to NOT include front seats.
 
Step 3: Have your insurance agent or company FAX a copy of your insurance policy showing the coverage amounts and the policy effective dates to Chadbourne at our FAX number, 510-656-6026. The cover should indicate it is for me so Bobbie knows to put it in my mailbox. (The reason it MUST be FAXed is because all insurance must be verified. Due to privacy laws, most insurance companies will no longer release your information to us over the phone. Therefore, you must call your agent or company and authorize them to FAX us a copy.)
 
Step 4: Make a copy of your California Driver's License being sure the expiration date is clear.
 
Step 5: Drop off the completed Declaration of Volunteer Driver Form and the copy of your driver's license to the office or me.
 
When I receive all three requirements I will send you the final paperwork and let you know that all is cleared and you're set to drive! And thank you!
