Senior Profile

This form is required for Seniors applying to colleges which require a counselor recommendation letter for admissions. These colleges are mostly private schools but may include public out-of-state colleges. This form is not sent to the colleges but is used by Mission San Jose staff to assist in the admissions process

This is an editable PDF. Please fill in as much as you can online (especially your name and contact information), then print it out. You may the use a pen to fill out the rest of the information.
Please Note: Once you close the document, your entries will be lost. Print the document out before closing it.
 

In order to successfully complete the Private School application process, students must complete each step outlined in detail here. It is recommended that you create folders/files for each school you are applying to, treating this process systematically.
